HART Hub Waterloo Region Integrated health, social and addictions support, including mobile clinics, outreach, crisis response and a 12-bed transitional housing program.
Mental Health and Addictions System Transformation Team More than 40 community leaders and people with lived experience working to align mental health and addictions services across the region, close gaps, and reduce duplication.
Cross-sector complex needs committee Advancing coordinated responses for people with complex needs, with a focus on frequent emergency department visitors.
Transitional housing pilot Follow-up health care for unhoused individuals discharged from hospital, reducing repeat emergency room visits.
Acquired Brain Injury screening Screening and services embedded across multiple organizations, including outreach workers and hospital staff.
Youth Wellness Hubs expansion Free youth recreation and mental health programming expanding into KW4 communities (Kitchener, Waterloo, Wellesley, Wilmot and Woolwich).
