Anyone who received a first dose and has an appointment booked for a second dose will have their appointment adjusted to the new time interval of 16 weeks between doses. You will be notified by your preferred method of communication about the appointment change in the coming days. You will receive a second notification with your new appointment date, time, and location. You do not need to book or reschedule your own second dose appointment.

Clinic locations and information
- These clinics are open for priority groups who have an appointment booked. No walk-ins will be accepted at these clinic locations. Pre-registration is available for priority groups to be contacted when an appointment becomes available.
- Due to limited supply, COVID-19 vaccination appointments are not available for the general public at this time. Clinics will open to the general public in Phase 3 of Ontario's vaccine rollout.
- Accommodation of a support person may be provided to those requiring assistance, but support persons will not be immunized and are not eligible to book an appointment at this time (unless they are also eligible in one of the priority groups).
- If you have symptoms of COVID-19, do not attend the clinic. Please call 519-575-4400 and follow their instructions.
- Plan to arrive no earlier than your appointment time. If you arrive early by vehicle, please wait in your vehicle until it is time for your appointment. Everyone who has an appointment will be vaccinated.

Individuals 16-17 years of age living in congregate care or developmental service settings or with highest and high-risk health conditions, must be vaccinated with Pfizer at this time. Please book at one of the following clinics:

Pop-up clinics for First Nations, Métis and Inuit residents
These pop-up clinics will open for booking for First Nations, Métis and Inuit residents who want to get the vaccine in a culturally appropriate setting. At both locations, an Elder will be available to support individuals with questions about health, culture and vaccine hesitancy. Appointments must be booked in advance. No walk-ins will be accepted.

- “Indigenous adult” and indicated “I am comfortable being vaccinated in any clinic setting” will receive a booking link from the Region of Waterloo to book an appointment at any of the existing clinic sites.
- “Indigenous adult” and indicated “I would prefer vaccination in partnership with an Indigenous organization” will be booked into one of the two First Nations, Métis and Inuit clinics.

Mobile clinic for homebound residents
Our goal is to ensure accessibility to vaccines for all residents. If able, eligible residents can book an appointment at any location to be vaccinated.
If you do not leave your home for personal, medical or social reasons - our mobile team can come to you. We ask homebound residents to email us with the following information:

Pharmacies offering vaccines
The Province of Ontario is working with the pharmacy sector to offer vaccinations at select pharmacy locations in Waterloo Region. You must have an appointment to get vaccinated at a participating pharmacy. Contact a participating pharmacy near you to book an appointment.
Primary care providers offering vaccines (not open to the general public)
The AstraZeneca vaccine is currently available at the following primary care office locations. Patients of these practices who are eligible for the vaccine need to book their appointment directly with their primary care provider. This particular COVID-19 vaccine is being offered to eligible individuals aged 55 and older.

How to cancel your appointment
- If you booked your appointment by phone, please call 519-575-4400.
- If you booked your appointment online, please use the “cancel” option in the confirmation email or text message you received.
- If you booked your appointment at a First Nations, Métis and Inuit clinic, please call 519-575-4400.
Before your vaccination
- Ensure you are in one of the priority groups that is currently eligible to book an appointment. Pre-register to be contacted when an appointment becomes available that you can book.
- Note: Clinic staff take vaccine prioritization seriously. Providing false information may result in cancellation of your appointment and notification to the appropriate regulatory college.
- Refer to information from the Ministry of Health about what you need to know before your COVID-19 vaccine appointment.
- Plan how you will get to your appointment. Please plan your trip so that you arrive at the clinic no more than 10 minutes early.
- If you need to use transit to get your scheduled vaccine, please visit the Grand River Transit website or call 519-585-7555 / TTY: 519-575-4608 for all current route information.
- Kiwanis Transit customers in Wellesley, Wilmot and Woolwich townships can book trips to vaccination clinics. Visit the Kiwanis Transit website for more information.
- Check your email or text confirmation for your appointment. It is important to ensure you have the correct clinic address and do not go to the wrong location. Clinics only stock the amount of vaccine needed for the number of appointments that are booked that day.
- If any of the following medical conditions apply to you, please contact your health care provider to discuss receiving the COVID-19 vaccine:
- have had a previous severe allergic reaction or anaphylaxis to COVID-19 vaccine or other vaccines
- have an autoimmune disease or a weakened immune system due to a medical condition or are on a medicine that affects your immune system
- you are pregnant
- Be prepared to attest that you have had a discussion with your health care provider about the risks and benefits of receiving the COVID-19 vaccine given you current condition or pregnancy.
- You should not receive the COVID-19 vaccine within 14 days of another vaccine (for example: flu shot, tetanus, etc.).

On the day of your vaccination
- If you have symptoms of COVID-19, do not attend the clinic. Please call 519-575-4400 and follow their instructions.
- Take your regular medication and eat your meals as usual (we recommend eating something before you come).
- Do not wear any scented products.
- Wear a loose-fitting top or a t-shirt so that the health care provider can easily access your arm for the vaccination.
- Wear your mask/face covering. If you do not have one, one will be provided to you at the door.
- Dress appropriately and for the weather. You may have to wait in line.
- Please plan on at least one hour of time at the clinic. We hope that most individuals will complete their appointment within one hour, however there may be some delays. Everyone with an appointment will get vaccinated.
- Bring your health card (OHIP) or any government-issued photo ID (if you do not have a health card) to confirm your appointment.
- If you do not have a valid health card, please bring an alternate piece of identification. This can be anything that confirms your identity and has a number/code assigned to it.
- For occupation-based priority groups, please bring workplace identification (e.g. workplace ID badge, letter from your employer) to confirm your eligibility.
- Check your email or text confirmation for your appointment. It is important to ensure you have the correct clinic address and do not go to the wrong location. Clinics only stock the amount of vaccine needed for the number of appointments that are booked that day.
What to expect when you arrive at the clinic
- All clinic staff will be wearing masks/face coverings and face shields. Sometimes it is difficult to hear under these conditions. Please ask staff to speak louder if you are unable to hear them.
- Visitors will be expected to follow all COVID-19 public health measures while in clinic. This includes physical distancing (at least 2 metres), wearing a masks/face covering and using provided hand sanitizer.
- Please arrive on time. This will help our clinics run as efficiently as possible and ensure physical distancing can be maintained. Please arrive no earlier than your appointment time. Everyone with an appointment will get vaccinated.
- Follow the instructions on signage at the clinic location.
- You will be screened for symptoms of COVID-19 prior to entry into the clinic.
- You will be asked for your health card (OHIP) or any government-issued photo ID to confirm your appointment.
- You will be asked to provide verbal consent to COVID-19 vaccination.
- Substitute decision-makers must be present at the clinic to provide consent, if applicable.
After your vaccination
- You will be asked to stay for 15-30 minutes after your vaccination to monitor for adverse reactions.
- Like all medicines and other vaccines, COVID-19 vaccines can cause side effects. The most commonly reported side effects do not pose a risk to health, are mild to moderate and should resolve in a few days. Examples:
- Pain, redness or swelling at the injection site
- Fatigue
- Headache
- Muscle or joint pain
- Chills
- Swollen glands
- Diarrhea
- Nausea/vomiting
- Enlarged lymph notes in your underarm
- Review the vaccine after care information sheet that tells you what to expect after vaccination.
- You will receive a record of immunization and instructions for getting your second dose, if applicable.
- Your second dose will be booked for 16 weeks after your first dose.

- This change reflects the updated guidance from the National Advisory Committee on Immunization (NACI).
- It is important to continue to follow the recommendations to prevent the spread of COVID-19, which include wearing a face covering, staying at least 2 metres from others and avoiding social gatherings with those outside your immediate household. Even once protected, you can still carry COVID-19 and pass the virus onto others.
