When applying for Child Care Subsidy, you will need to upload supporting documents with your application. The documents needed will vary depending on your circumstances and your reason for needing child care. All supporting documents need to be submitted within 30 days of your date of application to avoid the need to re-apply.
Your supporting documents can be submitted in one of two ways:
- Online - Upload all required documents directly to your Child Care Subsidy account within 30 days of submitting your application. This will avoid the need to re-apply.
- In-Person - Deliver your documents in-person or by mail to a Region of Waterloo Welcome Space where knowledgeable staff can also assist you in submitting your documents to complete your application. Please note, your application may be delayed if submitted by mail.
To prevent delays in processing your application, please carefully review the document information provided to ensure that all required documents are included and on time.
When your application and all documents are received and processed, you will be booked for an appointment with a Caseworker to complete the application process and assess your eligibility for Child Care Fee Subsidy.
Required Documents
Please ensure you are submitting the following required documents at one time. For more information about each, please read below.
Proof of your income
These documents show your household income. If you have not reported your income to the Canada Revenue Agency (CRA), we cannot determine if you are eligible for Child Care Subsidy.
Considerations: If you are new to Canada this year and have not yet filed your taxes, you are under 18 years of age, or if you are receiving benefits through the Ontario Works (OW) program, including Temporary Care Assistance (TCA), or through Ontario Disability Support Program (ODSP), you may apply without your most recent Income Tax Notice of Assessment (NOA).
Please provide one of the following:
- Most recent Canada Child Benefit (CCB) Notice. (Important note: Must show the tax year and Family Net Income)
- Most recent Income Tax Notice of Assessment (NOA). (Important note: Must show Line 23600, name, and tax year - One NOA is needed for each applicant in a two-applicant household)
- If you have received a Notice of Reassessment, or a Pre/Post Bankruptcy NOA, please include these documents as well
If you do not have your tax information available, you can access it through your account on the Canada Revenue Agency website or by calling:
Proof of identity and status in Canada
These documents show the legal name, date of birth, and, if born outside of Canada, the date of arrival and status in Canada for each applicant, and for each child requiring child care.
Please provide one of the following for each applicant and for each child:
If born outside of Canada:
- Canadian Citizenship Card or Certificate (front and back)
- Permanent Resident Card (front and back)
- Work Permit or Study Permit
- International Passport or Visitor's Permit (for child only)
- Record of Landing or Refugee Claimant Document
- Letter from Immigration, Refugees and Citizenship Canada (IRCC) verifying Permanent Resident status, application for status, or appeal status
- Visitor's Visa for Ukrainian Nationals
If born in Canada:
- Birth certificate, statement of live birth or certificate of adoption
- Ontario Driver's License or Ontario Photo Card. (Important note: Health card cannot be accepted)
- Valid Canadian Passport
- Indian Status Card
Proof of your Waterloo Region address
This document is used to determine that you currently reside in Waterloo Region.
Please provide one of the following:
- Most recent Canada Child Benefit (CCB)
- Most recent Notice of Assessment (NOA)
- Most recent utility bill (i.e. cable, hydro, or gas)
- Ontario Driver's License/Ontario Photo Card (Important note: Health card cannot be accepted)
- Most recent Lease/Rent agreement; Letter from Landlord (with address and names)
- Most recent mortgage statement/property purchase agreement/property tax statement
Note: If your spouse currently resides outside of Waterloo Region, please provide proof of your current address for both you and your spouse.
Proof of reason for child care
These documents show why you (and your spouse, if applicable) need child care. Each applicant must have an eligible reason for needing child care from the list below.
Working (Full-time or Part-time):
These documents show that you (and your spouse, if applicable) need child care because you are working.
Please provide at least one of the following for each applicant:
- Six weeks of your most recent pay stubs (e.g. three pay stubs if paid every two weeks). (Important note: Must show hours worked and name of your employer.)
- Employment Verification Form: If you have been with the company for less than 30 days or if you are returning to work after a leave, your employer needs to complete all sections of this form before it is submitted.
- Employer letter on company letterhead that is signed and dated, and must include all the following information:
- Name of Employee and Place of Employment
- Start date (and end date, if applicable)
- Weekly hours
- Supervisor's name, title, and work contact number
- Hourly wage
- Deductions (e.g. CPP, EI, Income Tax, etc.)
- Payroll calculator with hours and signed by your employer.
Working (Self-Employed):
These documents show that you (and your spouse, if applicable) need child care because you are working as self-employed.
Please provide all of the following for each applicant who is self-employed:
- Self-Employment Questionnaire for each self-employed applicant. If both applicants work for the same business, only one form needs to be completed.
- Most recent Income Tax Notice of Assessment (NOA) showing Line 15000 and tax year.
Also, provide one of the following:
- Master Business License; or Business Name Registration; or Official document stating HST number; or Certificate of Incorporation; or Independent Contractor Agreement.
- Courier and Rideshare programs (i.e. Uber, Lyft, Skip the Dishes, etc.) may provide a weekly/monthly Income Summary or letter/agreement or terms and conditions provided by the program.
Attending School or Training Program:
These documents show that you (and your spouse, if applicable) need child care because you are attending a school or training program.
Please provide at least one of the following for each applicant:
- School Letter of Acceptance or Letter of Registration
- Student Enrollment Form
- Tuition Statement
- Ontario Student Assistance Program (OSAP) Assessment Summary
- Other school-issued document with:
- Name of program and educational institution
- Name of student
- Full-time or part-time status
- Start and end dates of program
- Length of program (e.g. 2-year diploma, 4-year degree, etc.)
- Registration for scheduled professional licensing exam with date of exam
Important note: Must have applicant's name on any documents submitted.
Ontario Works (OW), Temporary Care Assistance (TCA), or Ontario Disability Support Program (ODSP):
These documents show that you (and your spouse, if applicable) need child care because you are receiving OW, TCA, or ODSP.
If you are currently receiving support from any of the programs above, please ensure that you’ve indicated that the Reason for Child Care in your application is "Ontario Works (OW), Temporary Care Assistance (TCA), or Ontario Disability Support Program (ODSP)" as further documentation may not be required.
Important Note: If you are receiving Ontario Works (OW), including Temporary Care Assistance (TCA), you can also connect with your Case Worker to complete a referral on your behalf. If you choose this option, you do not need to submit an application.
Therapeutic Referral - Child Special or Social Need:
These documents show that you (and your spouse, if applicable) need child care because your child has special or social needs.
If child care is required to support the special or social need of your child(ren), a child care referral is required.
- To obtain a referral for a child under 4 years of age, please contact Special Needs Access Point (SNAP) to schedule your assessment by visiting the SNAP website or calling 519-514-7627.
- A child care referral can also be submitted by a community agency or health professional who is working with your family. It is your responsibility to contact your referring agent to complete the Child Care Referral Form.
Therapeutic Referral - Parent Medical Need:
These documents show that you (and your spouse, if applicable) need child care because you have a parent medical need.
If you or your spouse require child care due to a medical need (e.g. illness, disability, stressor, or family circumstance), please send the Parent Medical Form to your doctor or other registered health professional to complete and sign. (Important note: This form is the only document accepted to verify your care needs. A doctor's note or report cannot be accepted.)
When you have a copy of the completed referral, please upload it directly to your Child Care Subsidy Account. The completed referral may also be emailed directly from your doctor or other registered health professional to ccs@regionofwaterloo.ca.
Parenting arrangement documents
These documents show that there is a parenting arrangement for one of your children.
If applicable, please provide one of the following:
- Court Order, Parenting Order or Parenting Plan (document verifying custody or parenting arrangement)
- Divorce or Separation Agreement; Mediation report
- Kinship Agreement
- Declaration of Family Composition Form
