Infection Prevention Lapse Reports
The Ministry of Health requires public health units to disclose when an infection prevention and control (IPAC) lapse is identified at a premises after an assessment or investigation. Assessments and investigations are initiated by complaints, referrals or communicable disease surveillance. Any current IPAC lapse reports identified by Region of Waterloo Public Health are at the end of this page.

A member of the IPAC team may contact you for further details. The information you submit will not be shared with the health professional or setting.

Public Health does not respond to complaints that are not related to IPAC, like:
- Wait times
- Diagnosis and treatment
- Conduct of a health care professional
- Patient service experience
Direct complaints that are not related to IPAC to the appropriate regulatory college or directly to the health care professional.

These reports are not exhaustive, and do not guarantee that those premises listed and not listed are free of infection prevention and control lapses. Identification of lapses is based on assessment and investigation of a premises at a point-in-time, and these assessments and investigations are triggered when potential infection prevention and control lapses are brought to the attention of the local medical officer of health.
