Public Health Dental Clinics

Public Health has two dental clinics, one in Cambridge and one in Waterloo. Dentists, Registered Dental Hygienists, and Certified Dental Assistants staff both clinics.

The clinics offer dental services to children and adults who:

  • Live in Waterloo Region
  • Meet the financial eligibility requirements 
  • Are not eligible for the Canadian Dental Care Plan (CDCP)

After submitting your eligibility assessment form, you may be asked to bring the following documents to the first dental appointment: 

  • Your and your spouse's (if applicable), Notice of Assessment or proof of income
  • Letter from Sunlife denying eligibility to the Canadian Dental Care Plan 
  • Information about any private dental benefits (e.g. dental coverage card)  
  • Proof of residency status if not a Canadian resident (e.g. student/work VISA document) 
  • A Healthy Smiles Ontario card, if enrolled

Children

Children 17 years or younger who meet the eligibility requirements receive free basic services, including checkups, preventive services (like scaling, fluoride and sealants) and fillings. 

To determine financial eligibility, call 519-575-4400 ext. 3086 or submit the Income Assessment form. Eligibility must be determined once a year and this form needs to be completed each time. If the application is approved, you will be contacted by dental clinic staff to set up an appointment for your child or children.

Financial eligibility is based on the family income. Those who are eligible have a combined household taxable income that is less than the limit for the family size - see chart below.

 Income levels established by Region of Waterloo Public Health

Adults

  • Adults 18 and older without social assistance may be eligible for temporary emergency dental coverage through discretionary benefits. To determine your eligibility, call Community Services at 519-883-2100 (option 1).
  • Adults 18 and older who are not eligible for social assistance or discretionary benefits may be eligible for limited services in public health dental clinics. Services are limited to extractions or fillings to eliminate pain and are accessible once per lifetime. To determine your eligibility, call 519-575-4400 ext. 3086 or submit the Adult Emergency Eligibility form.
  • Adults 18 and older may be eligible for the Canadian Dental Care Plan or, if 65 or older, the Ontario Seniors Dental Care Program.
