No Vaccine Record on File Letter

Dear Parent/Guardian:
The Immunization of School Pupils Act requires Public Health to collect and review vaccine records for all students in Ontario schools.
All students must be vaccinated against, or submit a valid exemption for, the following diseases: diphtheria, tetanus, polio, measles, mumps, rubella, meningococcal disease and pertussis (whooping cough). Students born in 2010 or later also need a varicella (chicken pox) vaccine.
Every time your child gets a vaccine from their health care provider, you need to notify Public Health. Your health care provider does not report vaccine records for you.
This letter is to inform you that Public Health does not have a vaccine record or valid vaccine exemption on file for your child.
